Initial session (~50 minutes)  $100  (6/1/2018 $140)
Individual session (~50 minutes)  $100 (6/1/2018 $130)
Couples session (~50 minutes) $125 (6/1/2018 $140)

Payment
Cash or check at start of session. Please make payments out to "Balanced Living Counseling, PLLC." I can provide forms to submit to HSA or FSA for your reimbursement. 

Discounted and Sliding Scale Rates
I believe that everyone should have access to quality counseling services.  Reduced fee schedules are available to those who need it.  Please feel free to discuss your particular situation.  I also offer a number of sliding scale sessions to clients who qualify based on income.

Cancellation Policy
If you need to cancel your appointment, please let me know by phone or by email at least 24 hours in advance.  I will bill for any missed sessions of cancellations with less than 48 business hours notice of a scheduled appointment.  The fee for the first missed appointment is $50 and the fee for subsequent missed appointments is $100 per appointment missed.  Please note, this fee is not covered by insurance.  The no show/late cancellation fee is due immediately.  By cancelling with adequate warning, you help others and yourself open appointments to make rescheduling easier.  

Insurance
Balanced Living Counseling is currently a network provider for Kaiser Permanente (formerly Group Health Cooperative), First Choice Health Network, Regence, and Blue Cross Blue Shield.  I also provide bills for several other insurance networks as an out of network provider, such as Premara and Lifewise.  Please ask me more for details.  As insurance plans become more complicated, I encourage all clients to speak directly with their insurance to gather information about authorization and benefits prior to our first appointment.

Balanced Living Counseling will continue to apply to be a network provider for several insurance companies, please check back regularly for updates.  Some insurance companies may reimburse you for out-of-network provider sessions.  Please check your coverage in advance if you are interested in out-of-network reimbursement.  I am happy to provide you with an invoice that you can submit to your insurance company for some reimbursement.  Several clients are currently using this method due to ease of access and therapeutic connection.  

Some common questions to ask your insurance company to see if reimbursement is possible:

Do I have mental health insurance benefits?
Does my insurance cover couples, family and group sessions?
What is my deductible and has it been met?
How many sessions per year does my insurance cover?
What is the coverage amount per session?
Is approval required by a primary care provider?  Do I need a referral?
Does my plan cover out-of-network coverage?
In what ways does my out-of-network coverage differ from my in-network coverage?
What is needed for both myself and/or my counseling provider in order to obtain reimbursement for the sessions?

Please be aware that in order to reimburse your health insurance companies may require that I provider certain information regarding your clinical diagnosis, treatment plans and summaries, or copies of your entire clinical record.  

Couples counseling is often not a covered service.  If you have an Employee Assistance Program (EAP) through work they often cover 3-6 sessions, you can call your HR department to explore this and their contracts.  Some couples also use their HSA (Health Savings Accounts) to get reimbursed for their payments.
